Please download and fill out all the information on all pages. If you have any questions or need help filling these out, please call us at (812) 967-3171 and choose option 2 to talk with a customer service representative. We will need a copy of a valid driver’s license that is not expired. Upon completion of the paperwork, you can bring it by the business office in Pekin at 105 E Railroad St. or email it to customer.service@telemedia.coop.   We will contact you with your account information. Prepayment of a $10 membership fee plus the amount for your first month’s payment will be due. We will then contact you for an appointment for your installation.

Starlight customers, we are now accepting new connection paperwork and your pre-payments for customers who are within the Grant Area. If you are not sure if you are inside the grant area, please call us to determine before completing and sending in your documentation. Once the construction is done, we will call to schedule a time for the install.
