Getting Started

Please complete the forms below. You will need to provide a picture of your valid driver’s license, so please have that ready! If you need assistance with completing these forms, do not hesitate to contact us at 812.967.3171.

Service Agreement – Must be completed by everyone. This is an agreement regarding equipment, weather, internet policies, etc.

Personal Info – Tell us about yourself! We will use this information to create your account. 

CPNI Notice – Security is a priority at TeleMedia. Let us know who we can speak to about your account! In order to verify authorized contacts, we will use the password you provide. Please share this password with your authorized contacts.

Letter of Agency – Only needed IF you choose to take phone service AND wish to have long-distance set up on your line.

Once your paperwork has been submitted, a customer service representative will reach out to regarding next steps.
