On behalf of the entire Telemedia staff, we’d like to take this opportunity to welcome you as a new customer. We are thrilled to welcome you as a new member into the Telemedia family! As a member owned cooperative our mission is to bring you the highest quality service and support at the lowest price possible.
Starlight customers, we are now accepting new connection paperwork and your pre-payments for customers who are within the Grant Area. If you are not sure if you are inside the grant area, please call us to determine before completing and sending in your documentation.
Once your paperwork is completed and entered into our software a customer service representative will reach out to you with your account information. A prepayment of $10 membership fee plus the amount of your first month’s payment will be due. This will get you all set up and get you on our construction list to have a service drop put to your house.
After the drops, our next contact with you will be to schedule an appointment for your actual installation of service.
The forms that are listed below will need to be completed in order for us to get you signed up for our service offerings. The forms can be downloaded and completed and brought by the office or you can complete them right here, right now, online. You will need to send a picture of your valid, not expired driver’s license, so please have that ready! Service Order – MUST BE COMPLETED BY ALL CUSTOMERS.
This lets us know your personal information. Physical and mailing address, where you work, contact information, what speed internet you want to sign up for, if you want phone service, etc.
Service Agreement – MUST BE COMPLETED BY ALL CUSTOMERS.
This is an agreement between Telemedia and you, the customer, regarding equipment, weather, internet policies, etc.
CPNI Notice – MUST BE COMPLETED BY ALL CUSTOMERS.
This provides security on your account. You let us know who we are allowed to talk to regarding your account and set a password so that we know we are talking to your authorized contacts.
Letter of Agency – MUST BE COMPLETED BY TELEPHONE CUSTOMERS FOR LONG DISTANCE.
